Health Insurance

  • Full-time and Part-time employees averaging over 20 hours per week. Coverage is effective on the first day of the month following date of hire

Dental Insurance

  • Full-time and Part-time employees averaging over 20 hours per week who elect coverage. Coverage is effective on the first day of the month following date of hire.

Vision Plan

  • Full-time and Part-time employees averaging over 20 hours per week who elect coverage. Coverage is effective on the first day of the month following date of hire.

Employee Life Insurance

  • Full-time and Part-time employees averaging over 20 hours per week.  Coverage is effective on the first day of the month following date of hire.
  • Proaxis Pays for life insurance on employee with coverage level of 1 x Salary.
  • Additional coverage is available for spouse and dependents.

Short Term Disability

  • Full-time and Part-time employees working over 20 hrs per week.
  • Short Term disability hours are accrued at a rate of 1.54 hours per pay period for a total of 40 hours Short Term Disability per year.

Long Term Disability

  • Full-time and Part-time employees working over 20 hours per week.
  • Long Term disability is offered through Reliance Standard with a 90 day elimination period.

401k Plan

  • Full-time and Part-time employees are eligible to contribute to the 401k program after 6 months of service and the company begins matching contributions after one year of employment.

Matching Contributions

  • Company matches 50% of the first 6% of pay you contribute each pay period.  You may contribute 1% to 100% of pay (subject to IRS rulings) on a pre-tax basis.  Company contributions (matching and retirement) are subject to the vesting schedule.

Vesting Schedule

  • Employees who have completed three years of employment are fully vested in the retirement plan.

Flexible Spending Account

  • Full-time and Part-time employees are eligible to enroll during open enrollment.
  • Medical and Vision: Contribution minimum – $10 monthly to maximum of $2000 per calendar year.
  • Dependent Care:  Contribution minimum – $10 monthly to maximum of $5000 per calendar year.

Supplemental Benefits

  • Cancer Policy, Short-Term Disability, and Accident Policy are offered

Continuing Education

  • Internal Professional Development: Internal courses will be scheduled 3-4 times per year based upon schedule demand and instructor availability
  • External Professional Development: Approved staff will be provided with opportunities to attend “external” professional development

Paid Time Off

  • All Employees pro-rated to FTE status
  • PTO is accrued each pay period based on years of service and hrs worked.  This benefit combines traditional vacation, holiday, and sick leave programs

Professional Organizations

  • Pay half of membership to professional organizations that relate to employment

Licensure

  • Pay for renewal of Colorado Licensure – not initial licensure
  • Pay for renewal of CPR card
